American Recovery and Reinvestment Act Forms and Resources

  • FY 2009/2010 ARRA Calculating Job Data Template (Excel)
    The ARRA calculating job data template must be completed in conjunction with the Quarterly Project Performance Report (QPPR) and is to be submitted via email to Contract_Manager@fcadv.org no later than the third day of the month following the end of each quarter. This template serves as a aide in determining the appropriate FTE count for reporting on the QPPR. The FTE calculation is a result of jobs created or retained under the American Recovery and Reinvestment Act. If the 3rd falls on a weekend, the template is due the preceding Friday. Each certified domestic violence center and/or legal service provider receiving ARRA funding must submit this form within the specified timeframe.
  • FY 2009/2010 ARRA CPI Budget (Excel)
    Line item budget template for certified domestic violence centers receiving ARRA funding. Funding amount may be entered by percentage into this template.
  • FY 2009/2010 CPI Monthly Project Report (Word)
    This report shall provide a brief description of the project, addressing each of the following areas monthly: Project partners participating; program progress; issues; and barriers to success.
  • Recovery Act Quarterly Project and Performance Report (Word)
    The Quarterly Project Performance report must be submitted via email to Contract_Manager@fcadv.org no later than the third day of the month following the end of each quarter. If the 3rd falls on a weekend, the report is due the preceding Friday. Each certified domestic violence center and legal service provider receiving ARRA funding must submit this form within the specified timeframe.
  • Recovery Act Quarterly Financial Report (Excel)
    The provider shall submit a Quarterly Financial Report documenting total expenditures for each quarter of the contract period. This report report must be submitted via email to Contract_Manager@fcadv.org no later than the 15th day of the month following the end of each quarter. The report shall document all expenditures by budget line item for the quarter and year to date, identifying federal funds, as well as actual amount spent versus amount budgeted.
